Part 4 Declaration - Ethics require a signed copy from each study site.
A signed copy of this must accompany the Locality Assessment form when presented for signing. There is a version adapted for ADHB use

For Non-ADHB researchers - you must obtain the signature of the Clinical Director from the service you will be recruiting participants from, not the HoD of your department. If the ADHB contact person is the clinical director, then please to the clinical leader or medical director for their signature. |

Full Ethics Application -

Please come to the Research Office on Level 14, Support Bldg, Auckland City Hospital on Mon, Tue, Wed and Fri.

Observational studies for Expedited Reviews - LAF is not required. However, you must notify Research Office of your study proposal and email documents to ResearchOffice@adhb.govt.nz to register your project and initiate the ADHB research approval process.

PLEASE NOTE: No research can begin if ADHB management approval has not been given. Management approval is NOT the locality assessment or ethics approval. Contact the Research Office if you are unclear whether you have ADHB management approval or not.
