 Access to Patient Records / Information for Research Purposes
 
Access to patient information for the purpose of research requires the ADHB research approval processes and ethics approval to be obtained the same way as other research activities do. There is a Board Policy: Access to Patient Information (December 2002) is not just for research but it is important that the ADHB policy on access are understood and complied with.  (This linked document is for internal ADHB access only, for external researchers please contact your ADHB contact or co-investigator for details).

For ADHB researchers, your current CRIS access will enable you to review the patient records for the purpose stated in your ethics application. If patient records are those not normally accessible with your CRIS permissions, these requests are to be actioned through Patient Information Services. Send a request to Clinical Records Manager to begin the process to extend your permissions.

If you are a non-ADHB researcher a request form needs to be completed. Once you completed this form, obtained the relevant signatures please send it to Research Office, Level 14, Building 1, Auckland City Hospital, Private Bag 92024, Grafton, Auckland.

Please note, in some circumstances a fee may be charged to access or retrieve records, particularly if retrieval of old records that are only in hard copy are required.
