Rotations at Each of the Hospitals

National Women's Middlemore North Shore Tauranga Whangerei Rotorua

National Women's Hospital

Orientation and Contacts: Orientation of Trainee Interns in Neonatal Paediatrics will be done by Dr. David Odd or Dr. Malcolm Battin and will take place in the Bonham Room after O&G orientation. Any additional questions can be directed to Dr. Odd, Dr. Malcolm Battin, or Diane Ambler (Secretary, Paediatrics, extension 3262).

Duties:

General: Trainee Interns will be scheduled to do a one-week rotation in Neonatal Paediatrics. Please report to the Neonatal Unit main desk at 8:30 a.m. on Monday and speak to the Level II consultant. Trainee interns generally follow the house surgeon or NS-ANP on acute delivery call but should attend Level II ward rounds at least three times during the week.

Ward rounds: Ward rounds begin daily at 8:30 a.m., except on Tuesday when rounds begin at approximately 9:30 a.m. after Grand Rounds.

Acute admitting: Trainee Interns are expected to do three on-call days (normally until 22:00) during their week on service. Generally these days are done between Monday-Friday; however, if two trainee interns are on during the same week then one trainee intern should do their call on Saturday.

Case presentations: If your attachment in Neonatal Paediatrics coincides with that of the 5th year students, you will be responsible for guiding and commentating on their case presentation session, which is held in the Bonham Lecture room, 2nd floor, O&G Department on Thursdays at 14:00-16:00. These sessions are moderated by one of the Neonatologists. Please contact Diane Ambler (extension 3262) for a list of the teaching topics so that you can read about these topics ahead of time.

Middlemore Hospital

Orientation and Contacts: Orientation of Training Interns will be done by Dr. Harshad Patel in the O&G Conference Room immediately after their O&G orientation unless otherwise specified by Chrissy Cochrane (O&G secretary for University of Auckland). Any questions can be directed to Dr. Harshad Patel (extension 8363) or Chrissy Cochrane (extension 8274).

Duties:

General: Trainee interns will be expected to do one week in Neonatal Paediatrics (Monday-Friday). Please report to the Special Care Baby Unit (SCBU) at 8:30 a.m. on the first day of your rotation.

Ward Rounds: Handover between the night registrar/NS-ANP and the day team starts at 8:00 a.m. in SCBU and formal rounds with the neonatologist begin at 8:30 a.m. Generally trainee interns will follow the registrar or NS-ANP on call for deliveries; you will also be expected to examine neonates under the supervision of a registrar or NS-ANP and should be prepared to present some babies by the middle of the week. In addition, trainee interns will spend one day, generally Tuesday, with the house surgeon on the postnatal floors. You should contact the Paediatric House Surgeon on duty at 8:00 a.m. that morning. The trainee interns can assist the house surgeon in attending to patients and attend elective C-sections with them. Formal rounds are made with the neonatologist on service after rounds in SCBU have been completed. Once work is completed on the postnatal floors, trainee interns can attend deliveries with the SCBU registrar/NS-ANP on delivery call.

Acute admitting: Trainee interns will be expected to do three long days (until 22:00), generally between Monday and Friday, during their rotation in Neonatal Paediatrics.

Case presentations: If 5th year students are in session you will be expected to guide and comment on their case presentations. These sessions are held on Thursdays from 2:30 p.m. to 4:00 p.m. in the O&G Lecture Room. Please contact Dr. Sneha Sood for a list of topics so that you can read about them before the teaching session.

North Shore Hospital

Currently all Neonatal Rotations for Training Interns on their O&G rotation are done at National Women's Hospital.

Tauranga Hospital

Orientation and contact information: Trainee Interns will be oriented to Neonatal Paediatrics during their O&G orientation. Dr. Hugh Lees can be contacted for any questions related to Neonatal Paediatrics.

Duties: Trainee Interns should check with the Neonatal Paediatrician daily to see if there are any interesting cases or procedures. Please discuss attendance at deliveries and any after-hours call responsibilities with Dr. Lees.

Whangerei Hospital

Duties:

During the 5 weeks on O & G you should examine the infants of your own obstetric patients as discussed in the section on general duties. Contact the Special Care Baby Unit (SCBU) and Paediatric SHO’s at the beginning of your attachment so that they can call you for any interesting cases, procedures and resuscitations.

During the paediatric week, you will have solely paediatric duties. On the first day of your attachment, contact the Charge Nurse and Senior House Officer covering SCBU.

(a) Ward rounds: attend each daily ward round, starting at approximately 1000hr. You should present at least 2 cases to the consultant on the ward round during your week in the nursery. Use the time before to familiarise yourself with all the cases on SCBU.

(b) Acute admitting: you will have rostered duties on 3 days during the week, each day from 0830 until the time of the evening handover (normally 2100), then from 0830 until at least 1100 the following day, so that babies admitted during the previous 24hr can be presented on the round. (Note that your attachment includes part of a weekend: if you are on call on Friday, attend the Saturday morning round; if on call on Saturday, attend the Sunday round.) You should admit, examine, work up and read around the problems of a reasonable number of babies admitted to Level II care (list them in the log at the end of this guide).

(c) Case presentations. Learning by teaching is one of the most effective methods of learning. You will be asked to present a topic to the Nursing staff and SHOs in SCBU with supervision from a consultant. If there are problems finding appropriate references, please contact the consultant on call early in the week.

Supervision

Your day-to-day contact will be the SHO, listed on the hospital roster on call for the day.

For the whole week, you will have a paediatric tutor, a consultant, commonly the one on call for much of the week. Find out who your tutor is SCBU charge nurse, get in touch with him or her early in the week and arrange a time for your tutor to review your progress. At the end of the week, ask the SHO on your team to sign your log sheet, then hand it to your tutor during your review session. It is your responsibility to ensure it is signed. It will form part of your total assessment in Paediatrics for the year.

Rotorua Hospital

Duties:

(a) Ward rounds: attend each daily ward round, starting at approximately 0815hr in SCBU. You should present at least 2 cases to the consultant on the ward round during your week in the nursery. Use the time before to familiarise yourself with all the cases on SCBU.

(b) Acute admitting: you will have rostered duties on 2 days during the week, each day from 0815 until the time of the evening handover (normally 2200), then from 0815 until at least 1300 the following day, so that babies admitted during the previous 24hr can be presented on the round. (Note that your attachment includes part of a weekend: if you are on call on Friday, attend the Saturday morning round; if on call on Saturday, attend the Sunday round.) You should admit, examine, work up and read around the problems of a reasonable number of babies admitted to Level II care (list them in the log at the end of this guide).
