Back to Guideline on Deaths in NICU
Below is a copy of the form letter sent out to families who have had a baby die in the NICU.
Dear
We were all very sad when your baby died in the nursery and extend our deepest sympathy. This letter is to invite you to return to National Women's Hospital to discuss with me what happened to your baby and what that means for the future. We normally offer such an appointment to families of babies who have died to allow them to ask questions and clarify any events surrounding the baby's death.
Please do not feel obliged to keep the appointment if you do not wish to at this time. You are welcome to cancel it or change it as you wish. You may wish to delay the appointment or to make contact in the future should new questions arise.
The appointment is enclosed. Please bring any friends or support family that you wish. I look forward to seeing you, or to your phone call to cancel or change the appointment as you see appropriate. (Phone 6389 - 909 ext 3212).
Yours sincerely
