- Axillae/rectal
temperature recorded 15 minutes after
each donor pack is commenced.

blood in/blood out on the Neonatal Unit Record of Exchange
Transfusion. Keep a
running total.

- Observe
for any changes in neurological status - drowsiness, irritability.

- Record
blood results on the Exchange Transfusion Results Sheet

- Baby
must remain on continuous electronic monitoring of vital signs for at
least two hours post transfusion or longer if baby’s condition is
not stable.
