PART 4 DECLARATION |

| The Part 4 is part of the National Ethics Application that is used by the investigator and head of the department/clinical director to declare to the Ethics Committee that to the best of their knowledge the application is accurate and the proposed research is appropriate to be conducted in the service area/department. |

| Who signs Part 4 Declaration Section 2 for the ADHB? |
- The Clinical Director of the Service where the study participants are located.

- If the Principal Investigator of the study is also the Clinical Director of the Service then section (2) is to be signed by the appropriate Clinical Leader/Medical Director.

- If a non-ADHB investigator please note you will also need a Part 4 Declaration signed by your institution's head of department or similar.
